Since 2006, REACHUP, Inc. has reduced the number of babies who die before their first birthday by 58% in the areas that we serve. This important and successful work is the result of collective impact-shared vision, goals, agenda and data- among community partners. Over the past 20 years, we have strategically and deliberately placed ourselves at the center of service delivery in the most vulnerable and marginalized communities in Hillsborough County.
REACHUP GROWTH Doula- provides an integrated continuum of personalized, prenatal, childbirth, doula, postpartum, and early parenting and breastfeeding support services for marginalized communities whose infants are at high risk for poor health, social and cognitive outcomes.
REACHUP Central Hillsborough Healthy Start- Provides home visitations by nurses or community health workers to women that are pregnant, new mothers and women between pregnancies in East Tampa. The program focuses on keeping women and their babies healthy.
REACHUP Pregnancy Medical Home- Provides maternal medical nurses or maternal health specialist at select doctors' offices in the community to focus on disease and chronic illness management, health risk reduction during pregnancy.
REACHUP GROWTH Stress Management- Conducts social support interactive groups to provide participants with stress management education.
REACHUP Nurse Family Partnership- Provides nurses for home visits to first time pregnancy or higher risk pregnancy persons. The nurse visits focus on supporting a healthy pregnancy, as well as child development, safety, and family self-sufficiency.
REACHUP GROWTH 24/7 Dad- Provides tools, strategies, and exercises to help fathers and father-figures be more involved, responsible and committed 24 hours a day, 7 days a week.
T.E.A.M. DAD Fatherhood Initiative of Florida- Individualized services to fathers/father-figures that aim to increase participation in home-visiting services and strengthen family and child well-being.
Affirming Fatherhood Conference- Annual event for fathers, educators, service providers and community members to celebrate fatherhood, hear best practices and the latest information about fatherhood.
